Family Practice & Counseling Network

Family Practice & Counseling Network (FPCN) is a non-profit network of Federally Qualified Health Centers (FQHC) that provides comprehensive primary care, behavioral health, and dental services to underserved populations in Philadelphia. As a program of Resources for Human Development (RHD), FPCN utilizes a nurse-managed model of care to integrate physical and mental health services within a community-based framework. The network operates several key sites, including the Abbottsford-Falls and 11th Street Family Health Services centers, offering a holistic approach to wellness. Their services extend beyond clinical care to include pharmacy services, social work, and specialized programs for chronic disease management.
