(R)-3-oxobutyl 3-hydroxybutanoate (OBHB) is a ketone ester and a potent ketogenic agent. It is being investigated as an oral supplement for the treatment of hyperketonemia in patients with Alzheimer's disease dementia. The drug works by increasing plasma β-hydroxybutyrate (βHB) levels, providing an alternative energy source for the brain when glucose utilization is impaired, a characteristic of AD. Clinical studies have shown that OBHB is well-tolerated and can lead to improvements in mood, behavior, self-care, cognitive function, and daily activity performance in AD patients. It is administered orally, typically mixed with flavored syrups to mask its bitter taste.
