(R)-minalrestat is a small molecule aldose reductase inhibitor belonging to the benzylisoquinoline class. It was developed as an experimental agent for the treatment of diabetic complications, particularly targeting impaired microvascular reactivity associated with diabetes. The drug acts by inhibiting aldose reductase, an enzyme involved in the polyol pathway that contributes to diabetic microvascular damage. Preclinical studies have shown that (R)-minalrestat can correct abnormal vascular responses in diabetic and galactosemic animal models, likely through mechanisms involving nitric oxide and membrane hyperpolarization rather than cyclooxygenase pathways. Despite its promising pharmacological profile, (R)-minalrestat has not advanced beyond early clinical development[1][5][6][8].
