(R,R)-fenoterol is the (R,R)-enantiomer and eutomer of the $\beta_2$-adrenoceptor agonist fenoterol. While fenoterol has historically been used as a racemic mixture (containing both (R,R) and (S,S) isomers) for the treatment of asthma and chronic obstructive pulmonary disease, the pure (R,R)-isomer is currently being investigated for novel therapeutic applications. It acts as a potent and selective agonist of the $\beta_2$-adrenergic receptor, triggering the activation of adenylate cyclase and increasing intracellular cyclic adenosine monophosphate (cAMP). Research led by the National Institute on Aging (NIA) has highlighted its potential in neurodegenerative diseases, particularly Parkinson's disease, where it has demonstrated the ability to reduce $\alpha$-synuclein levels. Additionally, (R,R)-fenoterol is being explored for its anabolic effects on skeletal muscle to treat muscle wasting (sarcopenia) and for its positive inotropic effects in the treatment of heart failure.
