(S)-propranolol is the levorotatory enantiomer of the non-selective beta-adrenergic receptor antagonist propranolol. While propranolol is typically administered as a racemic mixture, the (S)-enantiomer is primarily responsible for the drug's beta-blocking activity, possessing approximately 100 times greater affinity for beta-adrenergic receptors than the (R)-enantiomer. It acts as a competitive antagonist at both beta-1 and beta-2 receptors. Beyond its established cardiovascular applications, recent research has explored its potential antineoplastic properties. Studies indicate that (S)-propranolol can inhibit the MAPK signaling pathway and modulate the immune response by downregulating PD-1 expression on T cells and reducing the population of regulatory T cells (Tregs), thereby suppressing tumor growth in models such as colorectal cancer.
