[11C]PBB3 is a carbon-11 labeled radiopharmaceutical developed as a positron emission tomography (PET) imaging probe for the in vivo visualization of tau pathology. It belongs to the chemical class of pyridinyl-butadienyl-benzothiazoles and was designed to selectively bind to hyperphosphorylated tau protein aggregates, including neurofibrillary tangles (NFTs). Unlike some earlier tau tracers that primarily target the 3R/4R mixed tau aggregates found in Alzheimer's disease, [11C]PBB3 has shown the ability to bind to a wider range of tau isoforms, including the 4R tau aggregates characteristic of atypical parkinsonian syndromes such as progressive supranuclear palsy (PSP) and corticobasal degeneration (CBD). However, it is classified as a first-generation tracer and is known to exhibit significant off-target binding in the basal ganglia and cerebral white matter. Due to the short half-life of Carbon-11 (approximately 20 minutes), its use is typically limited to facilities with an on-site cyclotron.
