Overview

[11C]PBR-28 is a second-generation radioligand used in positron emission tomography (PET) imaging to measure the 18 kDa translocator protein (TSPO), formerly known as the peripheral benzodiazepine receptor. TSPO is expressed on activated microglia, endothelial cells, and immune cells. The primary use of [11C]PBR-28 is as a biomarker for neuroinflammation and microglial activation in various neurological diseases such as multiple sclerosis, Parkinson's disease, and Alzheimer's disease. Its binding affinity to TSPO can be affected by genetic polymorphisms (notably rs6971), which stratify individuals into low-, mixed-, or high-affinity binders. Compared to first-generation tracers like [^11C]-PK11195, [11C]PBR-28 offers improved signal-to-noise characteristics for brain imaging[3][4][5][6].
