[123I]I-(HE)3-G3 is a diagnostic radiopharmaceutical designed for molecular imaging of HER2 expression in breast cancer. It consists of a Designed Ankyrin Repeat Protein (DARPin), specifically the G3 variant with an N-terminal (HE)₃ tag, labeled with radioactive iodine-123. The drug binds to the human epidermal growth factor receptor 2 (HER2), enabling non-invasive visualization and quantification of HER2 expression in tumors using single-photon emission computed tomography (SPECT). This approach aids in patient stratification and monitoring response to therapy for both HER2-positive and HER2-negative breast cancers. The agent was developed by Tomsk National Research Medical Center of the Russian Academy of Sciences and is currently being evaluated in Phase 1 clinical trials for its safety, biodistribution, and imaging efficacy in patients with primary breast cancer[1][5][8].
