Overview

[124I]IV-14 is a radiolabeled ribonucleoside analog designed as a positron emission tomography (PET) imaging agent for hepatocellular carcinoma (HCC). It is a derivative of the UCK2-selective antitumor agent 3'-ethynyluridine (EUrd) and specifically targets uridine-cytidine kinase 2 (UCK2), a rate-limiting enzyme in the pyrimidine salvage pathway that is frequently overexpressed in solid tumors, including HCC. Upon entering the cell, [124I]IV-14 is phosphorylated by UCK2 into a monophosphate, which is then further phosphorylated and trapped within the cell, allowing for the visualization of UCK2 activity. Developed by researchers at Case Western Reserve University in collaboration with 3D Imaging LLC, the tracer aims to provide high tumor-to-background contrast for the diagnosis and management of HCC, potentially overcoming the limitations of standard [18F]FDG PET imaging.
