[127I]SGMIB-2Rs15d is a non-radioactive, stable-isotope version of a theranostic radiopharmaceutical designed to target HER2-overexpressing cancers. It consists of the camelid single-domain antibody fragment (sdAb) 2Rs15d, which binds specifically to Domain I of the Human Epidermal Growth Factor Receptor 2 (HER2). This binding site is distinct from those of trastuzumab and pertuzumab, allowing the drug to target tumors that have developed resistance to these standard therapies. The sdAb is conjugated via the residualizing prosthetic group N-succinimidyl 4-guanidinomethyl-3-iodobenzoate (SGMIB) to stable iodine-127. Developed by Precirix (formerly Camel-IDS) in collaboration with the Vrije Universiteit Brussel and Duke University, [127I]SGMIB-2Rs15d serves as a cold surrogate for its radioactive counterpart, [131I]SGMIB-2Rs15d, primarily for use in preclinical toxicity and safety studies to evaluate the protein-linker conjugate's profile at high doses without the confounding effects of ionizing radiation.
