[131I]ICF01012 is a melanin-targeting radiopharmaceutical compound developed for the diagnosis and treatment of metastatic melanoma. This agent combines the beta-emitting radioisotope iodine-131 with ICF01012, a quinoxaline derivative that specifically binds to melanin receptors. The mechanism of action involves the delivery of beta radiation directly to melanoma cells through melanin targeting. When [131I]ICF01012 binds to melanin in melanoma cells, the beta electrons emitted by iodine-131 destroy cancer cells while minimizing damage to surrounding healthy tissues due to their relatively short range of action[6]. Preclinical studies have shown that [131I]ICF01012 significantly suppresses tumor growth, increases tumor doubling time, reduces metastasis, and enhances survival in various melanoma models[1][8]. This targeted radionuclide therapy (TRT) represents a promising approach for patients with pigmented metastatic melanoma, particularly those with limited treatment options. The first-in-human clinical trial (MELRIV-1) began in 2018 to determine the recommended therapeutic dose and evaluate safety and efficacy[3][4][10].
