[14C]oxaliplatin is a radiolabeled diagnostic microdosing formulation of the platinum-based chemotherapy drug oxaliplatin. Developed by the University of California, Davis, it was investigated in a Phase 0/II clinical trial (NCT02077998) as a predictive biomarker tool for patients with metastatic breast cancer. The diagnostic approach involves administering a subtherapeutic 'microdose' (approximately 1% of the standard therapeutic dose) of oxaliplatin labeled with carbon-14. Following administration, tumor biopsies are analyzed using accelerator mass spectrometry (AMS) to quantify the formation of covalent drug-DNA adducts. This microdosing assay is designed to identify patients likely to be resistant to oxaliplatin therapy by measuring the drug's primary molecular interaction—DNA cross-linking—at a level that avoids systemic toxicity. Although the trial was completed, it enrolled only one patient due to low accrual and the program was subsequently modified to focus on other cancer indications.
