[177Lu]Lu-6A10-Fab is a radiolabeled antibody fragment designed for intracavitary radioimmunotherapy (iRIT) in patients with glioblastoma. It consists of the Fab fragment of the monoclonal antibody 6A10, which targets carbonic anhydrase XII (CA XII), a tumor-associated antigen overexpressed in glioblastoma cells. The Fab fragment is conjugated to the beta-emitting radionuclide lutetium-177 ([177Lu]), enabling targeted delivery of radiation to residual tumor cells within the resection cavity after surgical removal of glioblastoma. This approach aims to prevent or delay tumor recurrence by selectively irradiating cancerous tissue while sparing healthy brain structures. Early clinical experience suggests that fractionated intracavitary administration is feasible and safe, with no significant therapy-related toxicities observed in initial cases. A multicenter phase I trial is ongoing to further evaluate safety and efficacy[1][5].
