**[177Lu]Lu-OncoFAP-23 + L19-IL2** is an investigational combination therapy for the treatment of advanced or metastatic solid tumors that are positive for fibroblast activation protein (FAP). [177Lu]Lu-OncoFAP-23 is a radiolabeled small-molecule ligand targeting FAP, designed to deliver the beta-emitting radionuclide lutetium-177 directly to the tumor stroma for targeted radioligand therapy. L19-IL2 is a clinical-stage immunocytokine, fusing an antibody (L19) specific for the extradomain B of fibronectin (a tumor-associated antigen) with interleukin 2 (IL-2), thereby targeting IL-2 to the tumor vasculature to locally activate immune cells. Preclinical studies and early clinical trials have demonstrated that the combination of these two agents achieves synergistic anticancer activity through direct cytotoxicity of the radioligand and activation of natural killer cells and T cells, leading to enhanced tumor eradication. The combination is being developed and/or evaluated by Philogen S.p.A. with promising results reported in FAP-positive solid tumor models, including complete tumor remissions in animal models due to combined radioligand and immunostimulatory effects[1][3][4][6].
