[18F]-FLac is an experimental fluorine-18–labeled lactate analogue, chemically (±)-3-fluoro-2-hydroxypropionate, developed as a positron emission tomography (PET) radiotracer to image lactate uptake in tumors and other oxidative tissues.[3][7][8] It is transported into cells primarily via the inward monocarboxylate transporter MCT1, and its cellular accumulation decreases with pharmacologic or genetic inhibition of MCT1, enabling noninvasive monitoring of MCT1-dependent lactate uptake and its inhibition in vivo.[3][7] By probing lactate consumption in oxidative cancer cells that participate in metabolic symbiosis, [18F]-FLac is being investigated preclinically as a tool to stratify tumors and to assess response to therapies targeting lactate metabolism or monocarboxylate transporters.[3]
