[18F]AlF-FAPI-74 is a radiopharmaceutical diagnostic imaging agent used in positron emission tomography (PET) imaging. It is a fibroblast activation protein (FAP) inhibitor (FAPI) labeled with fluorine-18 via an aluminum fluoride ([18F]AlF) complex. FAP is a cell-surface serine protease that is highly expressed in the cancer-associated fibroblasts (CAFs) of the tumor stroma in various epithelial cancers, including gastric and esophagogastric junction adenocarcinomas, while showing low expression in normal tissues. This high tumor-to-background ratio makes it an effective tool for staging and detecting primary tumors and metastases. The tracer was developed to overcome the logistical limitations of Gallium-68 labeled tracers by utilizing the longer half-life and large-scale production capabilities of Fluorine-18. It is currently being evaluated in the PLASTIC-3 trial to assess its impact on the staging of advanced gastric cancer compared to standard laparoscopy.
