[18F]AlF-RESCA-T4 is an investigational radiopharmaceutical tracer developed by researchers at Renji Hospital, Shanghai Jiao Tong University School of Medicine, for the non-invasive visualization of Trophoblast cell-surface antigen 2 (Trop2) expression via Positron Emission Tomography (PET) imaging. The agent utilizes a Trop2-specific single-domain antibody (sdAb), also known as a nanobody, which is radiolabeled with fluorine-18 (18F) using the Aluminum Fluoride (AlF) restrained complexing agent (RESCA) method. Trop2 is a transmembrane glycoprotein overexpressed in various epithelial malignancies, including lung cancer, but minimally expressed in healthy tissues, making it a promising target for diagnostic imaging and screening for Trop2-targeted therapies (such as antibody-drug conjugates). The use of the RESCA chelator allows for efficient one-step radiolabeling with 18F, which provides a longer half-life (110 minutes) compared to gallium-68, facilitating high-contrast imaging and differentiation of malignant lesions from inflammatory conditions like tuberculosis.
