Overview

[18F]BCPP-EF is a fluorine-18 radiolabeled small molecule PET radiotracer designed to selectively bind to mitochondrial complex I (MC-I), also known as NADH-ubiquinone oxidoreductase. MC-I is the first and rate-limiting enzyme of the mitochondrial electron transport chain, making it a critical marker for mitochondrial density and function. Originally developed by researchers in Japan (associated with Hamamatsu Photonics), the tracer is used as a diagnostic imaging tool to evaluate brain energy metabolism and mitochondrial activity. It is currently being investigated in early-phase clinical studies for neurodegenerative conditions such as Parkinson's disease and Friedreich's ataxia, as well as for assessing brain mitochondrial activity in individuals with cocaine use disorder.
