Overview

[18F]fluoropropyl-trimethoprim is a radiolabeled small molecule diagnostic agent used for positron emission tomography (PET) imaging of bacterial infections. It is based on the antibiotic trimethoprim, which selectively inhibits bacterial dihydrofolate reductase (DHFR), an enzyme essential for DNA synthesis in bacteria. The fluorine-18 label enables sensitive and specific detection of live bacterial infections in vivo, distinguishing them from sterile inflammation or cancer. This agent demonstrates high uptake in live bacteria such as Staphylococcus aureus, Escherichia coli, and Pseudomonas aeruginosa, with low background signal in non-infected tissues. Developed primarily by the University of Pennsylvania, it is being investigated as a tool for noninvasive diagnosis and localization of bacterial infections using PET/CT imaging[1][2][3][4][8].
