Overview

[18F]PR04.MZ is a fluorine-18 labeled tropane derivative used as a positron emission tomography (PET) radiotracer for imaging the dopamine transporter (DAT). It is characterized by high affinity and selectivity for DAT, providing a more accurate assessment of presynaptic dopaminergic neuron density compared to earlier tracers. Clinical research has focused on its utility in the differential diagnosis of Parkinson's disease (PD) and atypical parkinsonian syndromes, such as progressive supranuclear palsy (PSP) and multiple system atrophy (MSA), particularly within the first five years of symptom onset. Notably, it is capable of visualizing dopaminergic loss in the substantia nigra pars compacta (SNpc), a feature that distinguishes it from many other DAT ligands.
