[68Ga]Ga-HN11-1 is a Gallium-68 labeled peptide radiopharmaceutical designed for positron emission tomography (PET) imaging of programmed death-ligand 2 (PD-L2). PD-L2 is an immune checkpoint protein that, along with PD-L1, regulates T-cell activity by binding to the PD-1 receptor. While PD-L1 is a well-established biomarker for immunotherapy response, PD-L2 expression has emerged as an independent predictor of efficacy for anti-PD-1 therapies, such as pembrolizumab, particularly in patients with non-small cell lung cancer (NSCLC). [68Ga]Ga-HN11-1 allows for the noninvasive, real-time, and whole-body quantification of PD-L2 expression, providing a more comprehensive assessment than traditional biopsy-based immunohistochemistry. It is currently being evaluated in preclinical models and initial human trials to stratify patients for immunotherapy and monitor treatment response.
