[68Ga]Ga-NOTA-PEG2-WP8 is an investigational peptide-based radiopharmaceutical designed for noninvasive Positron Emission Tomography (PET) imaging of Trophoblast cell-surface antigen 2 (Trop2) expression. Developed by researchers at Nanjing Medical University, the tracer consists of a Trop2-targeting peptide (WP8) derived from the natural ligand midkine (MDK) via computational simulation. This peptide is conjugated to the chelator NOTA (1,4,7-triazacyclononane-triacetic acid) through a PEG2 spacer and labeled with the positron-emitting radioisotope gallium-68. Preclinical evaluations in triple-negative breast cancer (TNBC) models demonstrate that the tracer has nanomolar affinity for Trop2, favorable pharmacokinetics, and high stability, allowing for rapid and specific identification of Trop2-positive tumors. It is primarily intended as a companion diagnostic tool to monitor Trop2 expression levels and assess tumor response to Trop2-targeted antibody-drug conjugates (ADCs) such as sacituzumab govitecan and datopotamab deruxtecan.
