**[89Zr]DFO-starPEG** is a novel diagnostic radiopharmaceutical developed for imaging purposes using positron emission tomography (PET). It consists of the radionuclide **zirconium-89 ([89Zr])** complexed by **desferrioxamine (DFO)**, which is conjugated to a multi-armed polyethylene glycol (starPEG) molecule. The tracer is designed to visualize **enhanced permeability and retention (EPR)-mediated uptake** in solid tumors. Its primary indication is to act as an imaging surrogate, specifically for mapping EPR effects in neoplasms before administration of EPR-based nanomedicines. Its mechanism is based on the passive accumulation in tumors via the EPR effect, allowing for whole-body PET imaging and dosimetric assessment in patients with malignant solid neoplasms and soft tissue lesions[1][5][7][8]. The agent’s biodistribution, safety, and pharmacokinetics are being characterized in first-in-human phase 1 clinical studies.
