[89Zr]Zr-DFO-Fab8 is a novel immuno-positron emission tomography (immunoPET) radiopharmaceutical designed for the noninvasive imaging of human CD68 (huCD68), a pan-macrophage marker. The tracer consists of a human-specific antibody fragment (Fab8), identified via phage display, which is conjugated to the p-SCN-Bn-DFO chelator and radiolabeled with the positron-emitting isotope zirconium-89 (89Zr). By targeting CD68, [89Zr]Zr-DFO-Fab8 allows for the visualization and quantification of macrophage infiltration within the tumor microenvironment (TME) and other inflammatory sites. Macrophages, particularly tumor-associated macrophages (TAMs), are critical components of the immune landscape that influence disease prognosis and therapeutic response. Preclinical evaluation in mouse models of triple-negative breast cancer has demonstrated that the tracer specifically binds to human macrophages in vivo, providing a potential tool for monitoring macrophage burden in various clinical conditions, including cancer and inflammatory diseases.
