[99mTc]Tc-ADAPT6 is a radiopharmaceutical imaging agent consisting of the engineered scaffold protein ADAPT6 labeled with the radioisotope technetium-99m. It is designed for noninvasive molecular imaging of human epidermal growth factor receptor type 2 (HER2) expression in tumors, particularly breast cancer. The mechanism involves high-affinity binding of the small (46–59 amino acids) ADAPT6 protein to HER2 on tumor cells, enabling visualization via SPECT imaging. This allows discrimination between HER2-positive and HER2-negative tumors and may help stratify patients for HER2-targeted therapies in settings where PET is not available. Phase I clinical studies have demonstrated that [99mTc]Tc-ADAPT6 is safe, well-tolerated, and provides clear tumor-to-background contrast for detecting HER2 expression[1][2][3][7][9].
