[99mTc]Tc-PSMA-HSG is a dual-labeled, small-molecule radiopharmaceutical designed for the intraoperative detection of prostate cancer through radio- and fluorescence-guided surgery (RGS/FGS). It targets the prostate-specific membrane antigen (PSMA) with high affinity (IC50 = 38.4 nM). The molecule is a hybrid tracer, incorporating the radioisotope technetium-99m (99mTc) for preoperative SPECT/CT imaging and intraoperative gamma-probe detection, alongside a cyanine-based fluorophore for real-time near-infrared fluorescence visualization. Developed as an optimized derivative of the PSMA-I&S scaffold, it exhibits favorable pharmacokinetics and high tumor-to-background ratios, facilitating the precise identification of lymph node metastases during surgical procedures.
