111In-PSMA-I&T is a radiopharmaceutical designed for the targeted detection and intraoperative localization of prostate cancer metastases. It consists of a peptidomimetic urea-based ligand (PSMA-I&T) that binds with high affinity to the extracellular domain of the prostate-specific membrane antigen (PSMA), which is frequently overexpressed in prostate cancer cells. This ligand is conjugated via a DOTAGA chelator to Indium-111, a gamma-emitting radioisotope. The primary clinical utility of 111In-PSMA-I&T is in radioguided surgery (RGS), where it enables surgeons to use a gamma probe to identify and resect small, otherwise occult lymph node metastases and soft-tissue lesions. Additionally, it can be used for preoperative SPECT imaging to map disease spread.
