16-[18F]fluoro-4-thiapalmitate (18F-FTP) is a small-molecule positron emission tomography (PET) radiopharmaceutical developed by Indiana University. It serves as a metabolically trapped analog of palmitate, designed to assess myocardial fatty acid oxidation (FAO) rates. The tracer's mechanism involves transport into the mitochondria via carnitine palmitoyl-transferase-I (CPT-1), where it undergoes partial beta-oxidation but becomes trapped, allowing for the quantification of fatty acid metabolism. It is primarily used as a diagnostic research tool to study myocardial fuel selection in conditions such as Type 2 Diabetes (T2DM), ischemic heart disease, and nonischemic cardiac diseases.
