177Lu-IMP288 + TF2 is a combination therapy used in pretargeted radioimmunotherapy (PRIT) for the treatment of cancers expressing carcinoembryonic antigen (CEA), such as colorectal cancer. The approach uses a bispecific monoclonal antibody (TF2) that binds both CEA on tumor cells and a hapten peptide (IMP288). After administration of TF2 to target the tumor, the radiolabeled peptide IMP288—conjugated with lutetium-177 (^177Lu)—is injected. The peptide rapidly binds to the prelocalized antibody at the tumor site, delivering targeted radiation while minimizing exposure to normal tissues. This two-step targeting system enhances tumor uptake of radioactivity and reduces toxicity compared to conventional radioimmunotherapy[1][4][5]. Preclinical studies have shown significant antitumor efficacy with limited toxicity in mouse models[1][4]. Clinical trials have evaluated this strategy in patients with CEA-expressing tumors including metastatic colorectal carcinoma and lung cancers[3].
