186Re-MAG3 + G250 is a combination radiopharmaceutical consisting of the monoclonal antibody G250 conjugated to the bifunctional chelator MAG3, which is radiolabeled with rhenium-186 (186Re). G250 is an antibody that specifically binds to the cell surface antigen carbonic anhydrase IX (CAIX), which is highly expressed in over 90% of clear cell renal cell carcinoma (RCC). The radiolabeled conjugate delivers targeted radiation to tumor cells expressing CAIX, combining the specificity of antibody targeting with the therapeutic effect of localized beta-emitting radioisotope. This approach is a type of radioimmunotherapy. The radiolabeling with 186Re provides both imaging and therapeutic beta emissions. Studies in animal models demonstrated tumor retention and therapeutic use, and clinical studies with analogous radioisotopes (131I, 177Lu) have been performed in RCC.
