**2,3-Dimercapto-1-propanesulfonic acid (DMPS)** is a small molecule chelating agent used primarily as an antidote for heavy metal poisoning. It acts by binding to heavy metal ions such as mercury and arsenic through its thiol groups to form stable water-soluble complexes that are then excreted from the body[6][4][5]. The sodium salt of DMPS is known as Unithiol. DMPS has been investigated for use in both acute and chronic intoxications by arsenic and mercury compounds[4], and has shown efficacy in reducing body burden of these metals in clinical settings[2]. It is related structurally to dimercaprol but offers improved water solubility. While it can lower urinary mercury levels effectively[2], it may not be effective at removing intracellular or brain-bound mercury. Additionally, preclinical studies suggest potential repurposing for early intervention after hemotoxic snakebite envenomation due to its metal-chelating properties[2]. Adverse effects are rare but can include severe reactions such as Stevens–Johnson syndrome.
