Drug intelligence / Profile preview
2-deoxy-2-[18F]fluorosorbitol (18F-FDS) is a radiolabeled sorbitol analog developed as a pathogen-specific positron emission tomography (PET) imaging tracer. It is synthesized via a simple, one-step chemical reduction of the widely available clinical tracer 2-deoxy-2-[18F]fluoro-D-glucose (18F-FDG). Unlike 18F-FDG, 18F-FDS is not accumulated by mammalian cells or sterile inflammatory lesions, but is selectively taken up and metabolized by certain microorganisms that utilize sorbitol as a carbon source. This includes Gram-negative bacteria of the Enterobacteriaceae family (such as Escherichia coli and Klebsiella pneumoniae) and clinically relevant molds (such as Aspergillus, Rhizopus, and Mucor). Consequently, 18F-FDS PET can specifically detect and localize bacterial and fungal infections, differentiating them from sterile inflammation and cancer. Additionally, because 18F-FDS is cleared almost exclusively through renal filtration, it has been investigated as a PET tracer for functional renal imaging and as a biomarker of blood-brain barrier (BBB) integrity.
