2HRbZE + 4HRb is a rifabutin-modified standard tuberculosis treatment regimen used as the backbone therapy in clinical research evaluating host-directed therapies for drug-sensitive pulmonary tuberculosis. The regimen follows standard TB treatment shorthand where '2HRbZE' denotes a 2-month intensive phase consisting of isoniazid (H), rifabutin (Rb), pyrazinamide (Z), and ethambutol (E), followed by '4HRb,' a 4-month continuation phase of isoniazid and rifabutin. In this regimen, rifabutin replaces the more common rifampicin to minimize drug-drug interactions with adjunctive agents, such as everolimus or auranofin, due to its lower induction of cytochrome P450 enzymes. Each component of the regimen targets Mycobacterium tuberculosis: isoniazid inhibits mycolic acid synthesis, rifabutin inhibits bacterial RNA polymerase, pyrazinamide disrupts membrane potential and energy metabolism, and ethambutol inhibits arabinosyltransferase.
