3-methoxy-4-(nitrooxy)ethoxy cinnamic acid is a synthetic small molecule and a nitric oxide (NO)-releasing derivative of ferulic acid. It was developed as a potential therapeutic agent for chronic kidney disease (CKD) to combine the antioxidant properties of ferulic acid with the vasodilatory and reno-protective effects of nitric oxide. In preclinical studies using adenine-induced CKD rat models, the compound (designated as 5a) demonstrated significant reno-protective activity by lowering serum blood urea nitrogen (BUN) and creatinine levels. Its mechanism involves the slow release of nitric oxide, which increases renal NO and superoxide dismutase (SOD) levels while reducing oxidative stress markers such as malondialdehyde (MDA) in kidney tissue.
