Overview

4SCAR19 + 4SCAR70 is a fourth-generation (4SCAR) dual-target chimeric antigen receptor (CAR) T-cell therapy developed for the treatment of relapsed or refractory B-cell malignancies, including primary central nervous system diffuse large B-cell lymphoma (PCNS-DLBCL). The therapy involves the co-administration of two distinct autologous T-cell populations: one engineered to target the B-cell antigen CD19 and the other to target CD70, a tumor necrosis factor receptor ligand often overexpressed in lymphomas. These 4th-generation CARs are characterized by a complex intracellular signaling domain comprising CD28, CD27, and CD3z, designed to improve T-cell expansion and durability. A critical safety feature is the inclusion of an inducible caspase 9 (iCasp9) suicide gene, which allows for the rapid elimination of the CAR T cells in the event of severe adverse effects such as cytokine release syndrome (CRS) or immune effector cell-associated neurotoxicity syndrome (ICANS). This dual-targeting approach is intended to overcome antigen escape, a common cause of relapse in single-target CAR-T therapies.
