ZK041 is a development code for 5-aminolevulinic acid (5-ALA), a small molecule metabolic precursor used as an optical imaging agent for the intraoperative visualization of malignant gliomas. Developed by Lee's Pharmaceutical, ZK041 is administered orally prior to surgery. Once ingested, 5-ALA is preferentially metabolized by tumor cells into protoporphyrin IX (PpIX), a fluorescent compound, due to altered enzyme activity in the heme biosynthesis pathway of malignant cells. Under blue light excitation (wavelength 375–440 nm) during neurosurgery, the accumulated PpIX emits a red-violet fluorescence, allowing surgeons to distinguish between malignant tissue and healthy brain tissue. This fluorescence-guided surgery (FGS) technique is designed to improve the extent of tumor resection and potentially enhance patient outcomes.
