This is a combination chemotherapy regimen consisting of four agents: - **5-fluorouracil (5-FU):** A pyrimidine analog antimetabolite that inhibits thymidylate synthase, disrupting DNA synthesis and function. - **Cisplatin:** A platinum-based alkylating agent that forms DNA crosslinks, leading to apoptosis in rapidly dividing cells. - **Folinic acid (leucovorin):** Enhances the binding of 5-FU to thymidylate synthase, increasing its cytotoxicity. - **Mitoxantrone:** An anthracenedione antineoplastic agent that intercalates into DNA and inhibits topoisomerase II. This regimen has been studied as adjuvant intraperitoneal chemotherapy for patients with stage II–III gastric cancer after curative resection. The combination aims to maximize cytotoxic effects on residual tumor cells within the peritoneal cavity. Toxicities are generally acceptable but include significant gastrointestinal side effects such as nausea and vomiting. The regimen is not currently recommended outside clinical trials due to survival rates comparable to no adjuvant treatment[1][3].
