This is a combination chemotherapy regimen consisting of three agents: - **5-fluorouracil** (a pyrimidine analog antimetabolite that inhibits thymidylate synthase, disrupting DNA synthesis), - **cisplatin** (a platinum-based chemotherapeutic that forms DNA crosslinks and induces apoptosis), and - **trastuzumab** (a monoclonal antibody targeting the human epidermal growth factor receptor 2 [HER2], inhibiting HER2-mediated signaling). This regimen is primarily used as first-line treatment for patients with HER2-positive metastatic adenocarcinoma of the stomach or gastroesophageal junction. The addition of trastuzumab to cisplatin and fluoropyrimidine-based chemotherapy has been shown to improve survival in this population. Trastuzumab specifically targets tumors overexpressing HER2. The combination may also be used in other upper gastrointestinal cancers expressing high levels of HER2[3][5][7].
