Overview

This drug is a combination chemotherapy regimen consisting of three agents: 5-fluorouracil, epidoxorubicin (an analogue of doxorubicin), and cyclophosphamide. It is most commonly referred to as the CAF regimen when doxorubicin is used, but can also be formulated with epidoxorubicin in place of doxorubicin. Each component has a distinct mechanism: - 5-fluorouracil (5-FU) is an antimetabolite that inhibits thymidylate synthase, disrupting DNA synthesis and repair. - Epidoxorubicin (epirubicin) is an anthracycline antibiotic that intercalates into DNA and inhibits topoisomerase II, preventing DNA replication and repair. - Cyclophosphamide is an alkylating agent that crosslinks DNA strands, leading to cell death. The combination attacks cancer cells through multiple mechanisms to maximize cytotoxicity. This regimen has been widely used as adjuvant or neoadjuvant therapy for breast cancer at high risk for recurrence[1][3][4].
