68-Ga RM2 + 68-Ga PSMA11 is a combination of two radiopharmaceutical PET imaging agents used in prostate cancer management. 68-Ga PSMA11 is a gallium-68–labeled ligand targeting the prostate-specific membrane antigen (PSMA), highly expressed in most prostate cancers, while 68-Ga RM2 is a gallium-68–labeled gastrin-releasing peptide receptor (GRPR) antagonist, targeting GRPR-expressing prostate cancer lesions. Both are administered intravenously for PET imaging to detect, localize, or guide biopsy of prostate cancer, especially in cases of suspected recurrence or initial staging. This dual-agent approach can increase the sensitivity of disease detection, as tumors may differentially express PSMA and GRPR. Their primary mechanism involves specific binding of the PET imaging probe to its respective target (PSMA or GRPR) on prostate cancer cells, allowing for improved lesion visualization via PET/MRI. 68-Ga PSMA11 is FDA approved for PET imaging, while 68-Ga RM2 is investigational and mainly used in research settings. The combination is under clinical investigation for enhanced diagnostic performance and biopsy guidance in prostate cancer.
