68Ga-FAPI-46 + 177Lu-EB-FAPI is a theranostic radiopharmaceutical combination developed for the management of pancreatic ductal adenocarcinoma (PDAC). It targets Fibroblast Activation Protein (FAP), which is highly expressed in the cancer-associated fibroblasts (CAFs) of the tumor stroma. 68Ga-FAPI-46 is used as a diagnostic PET tracer to visualize FAP expression and detect early metastatic disease or recurrence. 177Lu-EB-FAPI is a therapeutic agent that delivers beta-emitting radiation to the tumor site. The EB (Evans Blue) modification allows the molecule to bind reversibly to serum albumin, extending its circulatory half-life and increasing tumor uptake compared to standard FAPI molecules. This combination is being evaluated by Institut Curie for its potential to improve early diagnosis and treatment outcomes in pancreatic cancer.
