**68Ga-PSMA-I&T** is a **PSMA-targeted gallium-68 PET radiopharmaceutical** developed as the diagnostic member of the PSMA-I&T theranostic pair for **prostate cancer**. It is a low-molecular-weight urea-based ligand that binds **prostate-specific membrane antigen**, which is highly expressed on many prostate cancer cells, and the attached **gallium-68** radionuclide enables PET visualization of PSMA-expressing lesions after intravenous administration. The agent has been used clinically for prostate cancer staging, restaging, recurrence assessment, and selection of patients for PSMA-directed radioligand therapy, particularly in metastatic castration-resistant disease. The PSMA-I&T scaffold was reported from academic radiopharmaceutical development efforts centered at **Technical University of Munich** with collaboration including **Scintomics** and clinical investigators in Germany.
