This is a combination therapy consisting of 8-methoxypsoralen (also known as methoxsalen or 8-MOP), a photoactive psoralen compound, and an interferon (typically interferon alfa-2a). The regimen is primarily used in conjunction with extracorporeal photochemotherapy (ECP) for the treatment of cutaneous T-cell lymphoma (CTCL), particularly mycosis fungoides. In this protocol, oral 8-methoxypsoralen acts as a photosensitizer; after administration, blood is treated with ultraviolet A (UVA) light outside the body to induce DNA crosslinking and apoptosis in targeted lymphocytes. Interferon alfa-2a is administered subcutaneously to enhance immune modulation and antitumoral effects. This combination has demonstrated improved response rates compared to either agent alone in CTCL stage II patients[1][2]. The mechanism involves both direct cytotoxicity from DNA crosslinking by activated psoralen and immunomodulation via cytokine signaling from interferon.
