Overview

AAA817 + ARPI is a combination therapy under investigation for the treatment of metastatic castration-resistant prostate cancer (mCRPC) that is positive for PSMA (prostate-specific membrane antigen). AAA817 refers to [225Ac]Ac-PSMA-617, an actinium-225-labeled PSMA-targeted radioligand therapy. ARPI stands for androgen receptor pathway inhibitor, and refers in these trials to either enzalutamide or abiraterone, oral agents that inhibit signaling through the androgen receptor axis. The combination aims to pair targeted alpha particle radiotherapy (delivered by AAA817 to PSMA-expressing prostate cancer cells) with sustained hormonal pathway suppression by ARPI. The goal is to prolong radiographic progression-free survival in patients who have progressed following prior ARPI therapy and have not received taxane-based chemotherapy or earlier PSMA radioligand therapy. AAA817 + ARPI is being studied in a phase III, open-label, randomized, controlled trial led by Novartis Pharmaceuticals[1][2][3].
