abatacept + adalimumab

Abatacept + adalimumab is a combination of two biologic disease-modifying antirheumatic drugs (DMARDs) used off-label in the management of autoimmune and inflammatory conditions, most notably rheumatoid arthritis. Abatacept is a selective T-cell costimulation modulator that inhibits T-cell activation by binding to CD80 and CD86 on antigen-presenting cells, thereby blocking interaction with CD28 on T cells. This prevents the second signal required for full T-cell activation, reducing inflammation driven by autoreactive lymphocytes[4][5][7]. Adalimumab is a monoclonal antibody that binds specifically to tumor necrosis factor-alpha (TNF-alpha), inhibiting its interaction with TNF receptors and thus suppressing downstream pro-inflammatory signaling[8]. The combination targets two distinct immune pathways—T cell activation and TNF-mediated inflammation—to provide enhanced immunosuppression in cases where monotherapy or conventional DMARD combinations are insufficient. However, combining two biologics increases the risk of serious infections and is generally not recommended outside of clinical trials or highly refractory cases.
