Abemaciclib + endocrine therapy is a combination treatment regimen being evaluated for the adjuvant treatment of hormone receptor-positive (HR+), human epidermal growth factor receptor 2-negative (HER2-) breast cancer. Abemaciclib is a small-molecule inhibitor of cyclin-dependent kinases 4 and 6 (CDK4/6), which are critical regulators of the cell cycle. By inhibiting these kinases, abemaciclib prevents the phosphorylation of the retinoblastoma protein (Rb), leading to cell cycle arrest in the G1 phase. Endocrine therapy (ET), which may include aromatase inhibitors like letrozole, anastrozole, and exemestane, or selective estrogen receptor modulators like tamoxifen, works by reducing estrogen levels or blocking estrogen receptor signaling. The Japanese Foundation for Cancer Research is leading the Phase III AURA trial (JCOG2313) to determine if adding abemaciclib to standard ET improves invasive disease-free survival in patients who have undergone curative treatment for their first locoregional recurrence of breast cancer.
