Abemaciclib + trastuzumab + pertuzumab + loperamide is an investigational, multi-agent pharmaceutical regimen studied for the treatment of HER2-positive metastatic breast cancer. Abemaciclib is an oral small molecule inhibitor of cyclin-dependent kinase 4 and 6 (CDK4/6), leading to cell-cycle arrest in tumor cells. Trastuzumab and pertuzumab are monoclonal antibodies targeting the extracellular domain of the human epidermal growth factor receptor 2 (HER2), used to block HER2-mediated signaling and induce antibody-dependent cellular cytotoxicity. Loperamide, an antidiarrheal agent, is included prophylactically to manage abemaciclib-associated diarrhea. This regimen was investigated in a Phase 1b dose-escalation study; safety and tolerability were the primary endpoints, and efficacy for metastatic breast cancer was explored. The combination did not reach a recommended Phase 2 dose due to dose-limiting toxicities observed[1][4][5][6].
