Abiraterone acetate is an orally active, selective, and irreversible inhibitor of the enzyme **CYP17A1** (17α-hydroxylase/C17,20-lyase), which is essential for androgen biosynthesis in the testes, adrenal glands, and prostate tumor tissues. By blocking this enzyme, abiraterone significantly reduces serum testosterone to undetectable levels. It is co-administered with **prednisone**, a synthetic corticosteroid, to mitigate the compensatory increase in adrenocorticotropic hormone (ACTH) and subsequent mineralocorticoid excess (e.g., hypertension, hypokalemia) that occurs when CYP17 is inhibited. This combination is a standard of care for metastatic castration-resistant prostate cancer (mCRPC) and metastatic castration-sensitive prostate cancer (mCSPC). The University of Nebraska is currently evaluating this combination as an "enhanced ADT" regimen in a Phase 2 trial for prostate cancer patients who are PSMA-positive but show negative results on conventional imaging for pelvic lymphadenopathy.
