**Abiraterone decanoate + prednisone** is a combination therapy of a modified prodrug of abiraterone (decanoate derivative) with the glucocorticoid prednisone. Abiraterone is a CYP17A1 inhibitor that blocks androgen (testosterone) biosynthesis, resulting in decreased tumor growth in prostate cancer. Prednisone is coadministered to compensate for reduced cortisol due to CYP17A1 inhibition and to prevent or reduce mineralocorticoid excess side effects such as hypokalemia, hypertension, and fluid retention. This combination is indicated primarily for metastatic castration-resistant prostate cancer (mCRPC) and metastatic castration-sensitive prostate cancer (mCSPC), where abiraterone acetate plus prednisone is a well-established regimen; abiraterone decanoate may be a long-acting formulation, although clinical evidence and approval details for the decanoate form specifically are limited.
