Overview

Acalabrutinib + pembrolizumab is an investigational combination therapy consisting of two distinct agents: acalabrutinib, a next-generation, highly selective small-molecule covalent inhibitor of Bruton tyrosine kinase (BTK), and pembrolizumab, a monoclonal antibody targeting the programmed cell death protein 1 (PD-1) immune checkpoint. Acalabrutinib disrupts B-cell receptor signaling by inhibiting BTK, which is critical for the survival and proliferation of malignant B cells. Pembrolizumab blocks PD-1 on T cells to enhance antitumor immune responses by preventing tumor-induced T cell inactivation. The combination has been studied in phase 1/2 clinical trials for various hematologic malignancies and solid tumors such as head and neck squamous cell carcinoma (HNSCC), with the rationale that BTK inhibition may reprogram the tumor microenvironment to improve response to PD-1 blockade[1][2][5].
